1. Failure to first obtain written authorization prior to altering Nickel Acetate Seal (Tank 20) and Sodium Dichromate Seal (Tank 22), specifically by heating with propane instead of natural gas.

2. Operating an altered Nickel Acetate Seal (Tank 20) and Sodium Dichromate Seal (Tank 22) both propane heated without first obtaining a written permit to operate.

3. Failure to report to the Executive Officer the breakdown of the air pollution control equipment venting the Chromic Acid Anodizing Tank within one hour of the time said person knew or reasonably should have known of its occurrence.
