Rule 1148.2 Notification Form
This is the oil/gas well drilling, completion, or rework activity Notification Form to assist operators in complying with the requirements of Rule 1148.2(d)(1). Oil and gas well operators must use this electronic format (approved by the SCAQMD Executive Officer) to notify the SCAQMD prior to the start of well drilling, well completion, or well rework activities. Pursuant to Rule 1148.2(d), this notification shall be submitted to the District no more than 10 days and no less than 48 hours prior to the start of well drilling, well completion, or well rework. If the start date(s) of subject activities is modified, the operator shall notify the District at least 48 hours prior to the new start date if modified to occur earlier than the original start date. For well activities which occur after the originally projected start date and time, the operator may file extensions in 24-hour increments, up to a maximum five 24-hour extensions. If the well activity is not expected to occur within these timeframes, the operator shall cancel the original, or last revised (or extended) well event notification, and then apply for a new 48-hour notification.
Complete one Notification Form for each well where applicable activities (i.e., well drilling, well completion, well rework) will be conducted. Multiple well activities may be reported for one well, provided that the subject activities occur in series and the time between each activity in the series does not exceed 14 days. A separate report must be submitted for each activity in a series that occurs outside of the 14-day timeframe.
NOTE: Navigating away from the data input webpage (i.e., clicking the “Back” button or the “refresh” button) during the data entry process will delete any information that has not been saved. Also, if no user activity occurs on the data input page for more than 20 minutes, the system will automatically “log-out” without saving the information.
NOTE: The user should note that the new notification form has built in time frames prescribed by rule provisions that prevent you from submitting a form when the prescribed time deadlines have been exceeded (except for cancelation notifications). The previous notification form had no built in time restrictions so you could end up submitting a notification form that was in violation of the time deadlines. That is no longer the case.

Original: “Original” is for an initial notification for the well drilling, well completion or well rework on a specified well. Rule 1148.2 (d)(1).
Revision-Date: “Revision date” is to revise the start date and time of the well drilling, well completion, or well rework if an initial notification was submitted pursuant to Rule 1148.2 (d)(1) is modified to occur earlier than originally projected start date and time. Notes: Revision of site location address requires a new notification. The revision date notice must be submitted at least 48 hours prior to the start of the modified well event. Rule 1148.2 (d)(2).
Revision Other: “Revision-Other” is to update information such as “Operator name”, “Operator Phone”, or “sensitive receptor distance”, information in a previously submitted notification as well as updating selected well or well activity information.
Cancellation: “Cancellation” is to cancel a previously submitted notification where no new start date is planned within the 10 day notification period, or the well event exceeds the maximum number 24-hour extension periods, and a new 48-hour notice must be submitted. If a notification for a well drilling, well completion, and/or well rework is canceled, it must be canceled no later than the end of the 24-hour window of the most recently noticed start date and time. Rule 1148.2 (d)(4), (d)(5).
Emergency Pursuant to 1148.2 (d)(6): "Emergency Pursuant to 1148.2 (d)(3)" for well drilling, completion, or rework operations that are necessary to avert a threat to life, health, property, or natural resources. In these cases, notification shall be submitted no later than 48 hours after the start of the applicable operations.
Extension: “Extension” is used to extend the original projected start date and time by 24 hours. Up to five 24-hour extensions are allowed and the first extension begins at the completion of the 24-hour window following the originally projected start date and time. Each well activity event must begin by the end of the new 24-hour extended timeframe. Extensions must be submitted within and prior to the end of the 24-hour window (in the case of the first extension after the originally projected start date and time), or prior to the end of each successive 24-hour extension.
